MedicsPro is seeking experienced Health Visitors/ School Health Advisors (HV/SHA) - Band 6, who are looking for ongoing contract work in & around the London Area.
The role is Monday -Friday- 08.00 - 15.30 (Totalling 37.5 hours over the course of the week)
Job Summary:
The Health Visitor/School Health Advisor (HV/SHA) is responsible for the delivery of Universal Health Services to designated areas, Children's Centres and Schools. Reporting to the Team Leader they will be support the development of the service and work with colleagues to ensure the delivery of a safe and effective Universal Health Provision.
Key Responsibilities:
* Manage a caseload of patients, planning and delivering child health programs in
collaboration with families, schools, and communities.
* Identify health needs in families, schools, and communities, and deliver targeted health
promotion, illness prevention, and care programs.
* Support service development through clinical audits, standard setting, and evidence-based
care aligned with local and national policies.
* Act as the main contact for designated GP practices, developing collaborative
relationships to promote healthy outcomes for children and families.
* Build partnerships with schools and children's centres to coordinate care and support for
children, young people, and families.
* Collaborate with local providers to ensure accessible and inclusive services for all
community members, focusing on vulnerable groups.
* Promote effective teamwork, communication, and service integration within the healthcare
team.
* Ensure compliance with Trust policies and maintain accurate, up-to-date records.
* Maintain strong communication channels with healthcare staff and partner oorganisations
* Ensure a safe, healthy, and hazard-free environment for staff and patients, adhering to
infection control procedures.
* Encourage the use of up-to-date information technology to support clinical and service
improvements.
* Foster a continuous learning culture, providing access to relevant training and
information for safe and effective practice.
* Participate in audits, research, and innovative projects to improve service delivery.
* Support the training and development of new team members and nursing students.
